By submitting this form, I voluntarily assume all risks associated with participation and release South Bay Hard 2 Guard, its directors, coaches, staff, and affiliates from liability to the fullest extent permitted by law. I authorize program staff to seek medical treatment in the event of an emergency if I cannot be reached.
